Management/Service Coordination

Services provides contracted Case Management / Service
Coordination to individuals who receive funding through
Medical Assistance, Home and Community Based Waivers
including the Community Alternatives for Disabled Individuals
(CADI), Traumatic Brain Injury (TBI), Community Alternative
Care (CAC), Mental Retardation and Related Conditions
(MR/RC), Elderly Waiver (EW) and Alternative Care (AC).
Our Case Managers are experienced professionals who
work collaboratively with consumers and families to
ensure that needed services are implemented and that
services provided are adequate. Our goal is to provide
quality support services and to advocate on behalf of
the consumer to ensure that their needs and preferences
are met.
Our
Case Managers are licensed social workers that perform
all of the requirements stipulated by the State of Minnesota
including but not limited to:
• Assessments of individual’s needs
• Screenings
• Individual Service Plan development
• Community Support Plan development
• Identifying potential service providers
• Assisting the consumer in accessing needed services
• Coordination and monitoring service delivery
• Advocating on behalf of the consumer
