Case Management
Flexible
Case Management (FCM) is a service that can be purchased
through an individual’s Consumer Directed Community
Supports (CDCS) budget. Our Flexible Case Managers are
experienced professionals who work collaboratively with
families, individuals with disabilities, elderly individuals,
County and State officials, and other community agencies
to carry out the duties that are required when using the
Consumer Directed Community Supports option.
Pinnacle Services offers services to consumers who receive
funding under the Mental Retardation and Related Conditions
(MR/RC) waiver; the Elderly (EW) waiver; Community Alternative
Care (CAC) waiver; the Community Alternatives for Disabled
Individuals (CADI) waiver; and the Traumatic Brain Injury
(TBI) waiver. We are also offering these services to consumers
who receive funding under the Minnesota Disabilities Health
Options (MnDHO) and the Minnesota Senior Health Options
(MnSHO) programs.
Flexible Case Management is a viable option for families
and consumers who desire program or other technical support
when using the Consumer Directed Community Supports (CDCS)
option. All of the consumers we serve have the option
to choose from a menu of services.

Offer Families and Consumers
We work with families and consumers to provide support.
Areas of support include, but are not limited to:
• Developing a person-centered Community Support
Plan (CSP)
• Providing information about CDCS and provider
options
• Developing the Health and Safety Plan
• Facilitating community access
• Providing staff training that is specific to the
consumer’s plan of care
• Overall management of funds
• Identifying and accessing community services and
supports
• Assistance in recruiting, interviewing, hiring,
training, scheduling and paying employees
• Monitoring the CDCS plan and budget, and revising
it as needed
