Support Planning


Support Planning is a service that can be purchased through an individual’s Consumer Directed Community Supports (CDCS) or Consumer Support Grant (CSG) budget. Our support planners are experienced professionals who work collaboratively with families, individuals with disabilities, elderly individuals, County and State officials, and other community agencies to carry out the duties that are required when using the Consumer Directed Community Supports or Consumer Support Grant option.

Funding services
Pinnacle Services offers services to consumers who receive funding under the Developmental Disability (DD) waiver; the Elderly (EW) waiver; Community Alternative Care (CAC) waiver; the Community Alternatives for Disabled Individuals (CADI) waiver; and the Traumatic Brain Injury (TBI) waiver Consumer Support Grant (CSG). We also offer these services to consumers who receive funding under the Minnesota Disabilities Health Options (MnDHO) and the Minnesota Senior Health Options (MnSHO) programs.

Practical options
Support Planning is a strong option for families and consumers who desire program or other technical support when using the Consumer Directed Community Supports (CDCS) or Consumer Support Grant (CSG) option. All of the consumers we serve have the option to choose from a menu of services. We work with families and consumers to provide support in areas including:
 
·                     Providing information about CDCS or CSG and provider options
·                     Developing a person-centered Community Support Plan (CSP)
·                     Developing the Health and Safety Plan
·                     Identifying and accessing community services and supports
·                     Overall management of funds
·                     Facilitating community access
·                     Assistance in recruiting, interviewing, hiring, training, scheduling and paying employees
·                     Providing staff training that is specific to the consumer’s plan of care
·                     Monitoring the CDCS or CSG plan and budget, and revising it as needed
