Case Management

Pinnacle Services provides contracted Case Management/Service Coordination to individuals who receive funding through Medical Assistance, Home and Community Based Waivers including the  Developmental Disability (DD), Community Alternatives for Disabled Individuals (CADI), Brain Injury (BI), Elderly Waiver (EW), and Alternative Care (AC) programs. We also provide Care Coordination through Medica for consumers on the MSHO/MSC+ and SNBC programs.

Pinnacle Services’ case managers and care coordinators are all licensed social workers, who develop person centered support plans that effectively connect resources and services to each consumer. Our case managers are advocates for the consumers to live their lives to the fullest potential with services tailored to their individual needs. Pinnacle Services responds to all consumer service needs, questions, and concerns within 24 hours of contact.

Each consumer will receive case management services to locate informal and formal community resources; engage in face-to-face reassessments and additional team meetings to develop person-centered service plans that include vocational, behavioral, and psychosocial components; and provide service monitoring, evaluation, and referrals to appropriate resources to ensure health and safety needs are maintained within the community.
