Case Manager; LSW required

Pinnacle Service Inc. has a need for a Case Manager in Minneapolis, MN. This is a full-time opportunity and compensation for this role is $44,000/annually.

Case Manager Job Description

Case Managers have the opportunity to work with consumers that have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability or are aging. The Case Manager is responsible for assessing the areas of the consumer’s life where assistance is needed; offering and coordinating resources for the consumer; monitoring service delivery and working with providers to ensure the consumer’s needs are met. The Case Manager will do this through service plan development and regular meetings with consumers and their interdisciplinary teams. Successful Case Managers enjoy job tasks that vary from day to day, having a diverse array of clients, and having ownership of a caseload with ample opportunity to consult and collaborate.

Case Manager Requirements:

Applicants must be a Social Worker licensed by the Minnesota Board of Social Work. Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance. Applicants must successfully clear local and/or stated background checks.

Employees have the option to work from home outside of trainings and meetings 

Preferred Qualifications: One to two years’ experience in service coordination or case management is preferred.
