Position: Pinnacle Services is seeking a Full-time Program Manager for our Independence Program (Independent Living Services, Supported Employment Services, In Home Services, Home Care, Ryan White, Personal Support, Homemaker and Respite) serving children and adults with developmental disabilities and mental health diagnoses.

Job Summary: Specific responsibilities include supervision of direct care employees, weekly communication with case managers and guardians, assurance that staffing is in place for consumers on the case load, completion of all employee related paperwork, and completion of paperwork for assigned consumers. The Program Manager is responsible for scheduling and conducting annual meetings for each consumer on the caseload. Includes ensuring DHS (245D) licensing compliance, maintenance of CARF standards, quality assurance of overall agency objectives as well as individual consumer programming and team satisfaction with service delivery, assuring development and implementation of individualized consumer programs, and training of direct care staff in daily duties. This position will also include direct care in addition to management duties. Applicant must be able to travel to various Twin Cities areas as needed.

Requirements:

  • Must meet Designated Managers qualifications as defined by 245D:
  • A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older
  • An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
  • A diploma in a field related to human services from an accredited post secondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
  • A minimum of 50 hours of education and training related to human services and disabilities; and
  • Four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
  • Valid Minnesota Driver’s License with acceptable record required.
  • Available to work flexible hours and locations.
